After you complete the survey, your name will be entered into a draw to win a prize pack, including:

We will hold a random draw from the responses and will notify the winner via email on Saturday, May 9 by 10 a.m. with details on how to receive the prize. If we do not hear from the winner within 48 hours we will draw another winner.
